Home Health (RN) Case Manager - Full Time

Due to expansion, and significant growth our Pediatric & Adult Skilled Home Care Division is seeking an aggressive candidate for a skies the limit opportunity. Essential Job Functions/Responsibilities:

• Receives referrals, determines services required, and Agency’s ability to meet needs for pediatric and waiver patients.
• Supervisory visits to observe clinical skills and ensure exemplary client care and compliance
• Monitors documentation of pediatric and wavier patient clinical records to ensure compliance with regulatory standards of timeliness, accuracy, and completeness.
• Manages/supervises a team of RNs, LPNs, Home Health Aides to provide effective and quality home care services

Qualifications:

• Prior experience in scheduling/human resources/customer service experience required.
• Current license as a Registered Nurse in Pennsylvania.
• Demonstrated organizational and time management skills, excellent communication skills, interpersonal skills, documentation skills critical thinking, and ability to work independently
• Ambition to grow and advance with company goals.
• Excellent communication and customer service skills
• Must be able to multi-task in a fast paced environment
