Job Title: Registered Nurse, RN - Hospice Admissions/ Community Liaison

Monday thru Friday 8:30am to 5:00pm

Job responsibilities include, but not limited to the following:

• Perform start of care visits on hospice patients and admit patients to Traditional Home Health Care, completing clinical documentation as required

• Initial evaluations for hospice appropriateness

• Obtaining consents during initial hospice discussion with patients and families

• Assesses patient status and develops a plan of care with coordination of nursing supervisor, physician, patient and family members

• Coordination of hospice referrals including obtaining records/orders/diagnosis

• Maintenance of electronic hospice contracts

• Assist with transition of care between palliative program and hospice program

• Hospice philosophy teaching

• Coordination of care audits with our SNFs

Requirements:

• Current license as a Registered Nurse in Pennsylvania

• Excellent assessment skills, documentation skills and ability to meet deadlines

• Must have a current drivers license

• Ability to travel to case assignments

• Ability to work independently with minimal supervision and manage time effectively

• Strong interpersonal skills

• Demonstrates good verbal and written communication, and organizational skills

• Acute care and/or home health experience is preferred
